These documents contain general enrollment information including an enrollment form, a student information sheet, and links to group class schedules (Musicianship Classes and Group Piano Classes). The documents also provide important information about our fees and Conservatory policies.

Please complete both forms and fax or mail them to us so that the information is entered into our system and the student(s) is/are enrolled.
